What is Heartmoves?
The Heart Foundation’s Heartmoves program is a gentle physical activity program suitable for people of all shapes and sizes. Whether you are new to exercise or returning after illness or injury, Heartmoves allows you to work at your own pace in a friendly, supportive and safe environment.
Heartmoves is run by accredited professionals specifically trained in delivering low to moderate intensity physical activity programs for people with health conditions such as heart disease, diabetes and obesity.

All Heartmoves classes delivered in Southwest Victoria as part of the Eat Well Be Active Program cost no more than $5
An electronic copy of the Heartmoves Pre-Exercise Assessment and Referral Form can be found below. Please complete the form and if you tick “Yes” to any of the medical conditions listed in section 3, it is advised that medical clearance be obtained from your doctor prior to exercising (take the referral form with you).
The completed Referral Form is to be given to the Heartmoves Leader prior to the first class.
