Participation and Follow Up Form
Dear Residents of Warrnambool, Corangagmite and Moyne,
Your're invited to join the programs and activities of the Eat Well Be Active - Southwest Vic program by completing the participation and follow up form below. The programs are open to all adult residents and there is a particular emphasis on involving people with a disability and carers. We recommend that interested participants read and answer the questions together with their support worker or carer, as required.
The online survey is the quickest and easiest method to register your interest in the program or you can click below to download a printable version of the form. By indicating your interest in a particular activity our EWBA team will contact you about the activities that match your interests.
